- Email Address
- Shipping Address: First and Last Name, Street Address, City, State / Province / Region, Postal (Zip) Code, Country, and Telephone Number
- Billing Address: Please enter this info exactly as it appears on your credit card statement: First and Last Name, Street Address, City, State / Province / Region, Postal (Zip) Code, Country, and Telephone Number
- Credit Card Information: Credit Card Number, Name on Card, CSC Number (the 3 or 4 digit credit card security code), and Expiration Date
