I know its pretty simple and I could probably just lay one out from scratch but I thought I would ask.

Troy, Here's one to get you started:
Add seam allowances all around. End the footbox with a small fabric wedge between the second and third baffle.

Trickiest part is getting the footbox 'end piece' the right dimensions. PM me if you'd like a full size PDF pattern of the end piece.
