Evernew platy-style water bladders have a cap that is attached to the bottle. No more lost caps. Gossamer Gear has them http://gossamergear.com/etc/hydration/evernew-water-bladder.html

ULA Ohm
+1 on the ULA Ohm. No weight on my shoulders. The shoulder straps simply keep the pack from falling away from your back. All the weight is transferred to your hips.
